Prepare for a visit

For new patients, we request that you download, fill out, and bring the client and patient information forms with you to the appointment. Depending on the type of appointment, we may request medical history or adoption documentation. For annual exams, we ask that you bring a fresh fecal sample from your pet. Arriving prepared for the appointment will ensure that your pet is seen in a timely and thorough manner.

We also ask that all dogs and cats either be kept on leashes or secured in carriers while in the lobby for their safety.
